Jack's real name is exactly that, Jack Noir. Scratch is referring to the fact that he's going by Spades Slick now. And when he's talking about his own name, he's referring to the fact that he is actually a vessel for Lord English.

I always took it to mean Lord English, through his composite parts Equius and Gamzee. Remember that it was Equius who failed to stop Gamzee, and then Gamzee who killed her.
